Speech

With conventional braces on the outside of the teeth, speech changes very little. Some patients notice a slight lisp for two or three days while the tongue adjusts to the extra bulk at the gum line, and it resolves without doing anything. Lingual braces are a different matter — they sit where the tongue works, and adaptation takes two to four weeks. If you speak publicly for a living, that is worth planning around and worth raising before you choose a system.

Appliances that cover the palate, such as expanders, produce more noticeable changes for the first week. Reading aloud for ten minutes a day genuinely accelerates the adjustment, for any appliance.

Photographs

Braces catch flash, which is why they look more prominent in photographs than in a mirror. Practical things that help: choose darker-toned bands or silver-grey before a photography date, book the adjustment visit for fresh ligatures in the week beforehand, and turn slightly off-axis to the camera rather than facing it square. Or ignore all of it, which is what most people arrive at within a few months.

Teenagers

The first fortnight is usually the hardest, and by the second month braces have become unremarkable to everyone including the wearer. Two things reliably help: letting the choice of colours be genuinely theirs, and framing the finish date as something they control through elastics and looking after the appliance. A patient who feels like a participant rather than a subject wears their elastics.

Adults

Adults tend to worry about professional perception and then find it is a non-issue after a fortnight. If it is not, that is a legitimate reason to look at ceramic or lingual options rather than to postpone treatment for another five years. Postponement has a cost: crowding tends to increase with age, and gum and bone conditions do not improve.

The average patient stops noticing their braces at around three weeks. The average onlooker never notices them at all.
