The crowding claim

The theory is intuitive: third molars erupt at seventeen to twenty-five, lower front crowding often appears in the same window, therefore one causes the other. Studies comparing people with and without wisdom teeth have not found the difference the theory predicts. Late crowding occurs in patients whose wisdom teeth never developed, and it occurs in patients whose wisdom teeth were removed years earlier.

The likelier explanation is the combination of continued jaw growth, natural forward drift and soft tissue pressure, all of which act regardless of what is happening at the back. Third molars may contribute a small amount; they are not the driver.

Removing wisdom teeth to prevent front tooth crowding is not supported. Removing them for the reasons that actually justify it is a different question.

Reasons that do justify removal

• Repeated infection around a partially erupted tooth.
• Decay in the third molar or in the second molar next to it, where cleaning is impossible.
• A cyst or pathology associated with an unerupted tooth.
• An impacted tooth angled into the root of the second molar.
• Insufficient room for eruption in a position that will predictably cause problems.

Timing around orthodontics

Usually after. Most orthodontic treatment finishes before wisdom teeth are a factor, and they are assessed at debond and monitored on the radiographs taken during retention checks. There are exceptions: where treatment involves moving molars backwards, third molars are often removed first because they are in the way.

Removal is done by an oral surgeon or your general dentist rather than here, and we will coordinate the timing so it does not interrupt active treatment.
