White spot lesions

The one outcome of orthodontic treatment that does not resolve on its own. It is also the one most completely within the patient's control.

What they are

A white spot lesion is early decay. Plaque sitting against enamel produces acid, acid dissolves mineral out of the surface layer, and the demineralised area scatters light differently from healthy enamel — which is why it reads as a chalky white patch. Around a bracket it forms a distinctive square outline, invisible until the bracket is removed.

They can develop remarkably quickly. Visible demineralisation has been documented within a month of bonding in patients with poor hygiene and frequent sugar intake.

Why braces make it more likely

Brackets and wires create dozens of stagnation sites that a toothbrush does not reach without deliberate technique. The gum margin above each bracket is the highest-risk zone because it is the hardest to clean and the plaque there is least disturbed by the tongue and cheeks.

Prevention, in order of impact

  1. Interdental brushes under the wire, daily. The highest-value habit available.
  2. Brushing angled at the top and bottom edges of every bracket, twice daily, properly.
  3. Fluoride toothpaste, spat out rather than rinsed away.
  4. Cutting the frequency of sugar rather than the total quantity. Sipping a sweet drink across an afternoon is worse than a dessert at once.
  5. A fluoride mouthrinse at a separate time of day for patients at higher risk.
  6. More frequent cleanings with your general dentist during treatment, not fewer.

If they have already formed

Shallow lesions often improve over six to twelve months as saliva remineralises the surface, and high-fluoride or calcium phosphate products can help. Deeper ones do not fully resolve, and options then include resin infiltration, microabrasion, or restorative treatment with your general dentist. All of that is more expensive and less satisfying than an interdental brush.
