Gaps between teeth
Closing a space is often the quickest orthodontic treatment there is. Keeping it closed is the part that requires thought.
Why gaps appear
- Teeth smaller than the arch. The mirror image of crowding, and equally inherited.
- A prominent labial frenum. The band of tissue between the upper lip and gum, where it extends low between the front teeth, can hold a midline gap open.
- Missing teeth. Congenitally absent lateral incisors are relatively common and produce characteristic spacing.
- Teeth lost in adulthood. Neighbouring teeth tip into the space and the opposing tooth over-erupts into it.
- Tongue thrust. Sustained forward pressure can space the front teeth apart.
What closes easily
A simple midline gap in an otherwise good bite is among the most satisfying cases in orthodontics — often six to twelve months. Generalised mild spacing across an arch is similarly straightforward. Where a low frenum is holding the gap, a minor procedure to release it is sometimes done alongside treatment, usually after the space has been closed rather than before.
What is more involved
Missing lateral incisors present a genuine choice: close the space entirely and reshape the canines to look like laterals, or open the space properly and restore it with an implant or bridge. Each has real trade-offs in appearance, cost and timing, and implant placement generally waits until growth is complete. That decision is made jointly with your general dentist and is worth taking time over.
Spaces from teeth lost years ago also need the neighbouring teeth uprighted and the over-erupted opposing tooth intruded before anything can be restored, which is slower.
Retention
Closed spaces have a stronger tendency to reopen than crowded teeth have to re-crowd, because the elastic fibres in the gum retain a memory of the original position for years. Spacing cases almost always finish with a bonded wire behind the teeth rather than a removable retainer alone. This is not optional in these cases, and anyone who tells you otherwise has not seen enough of them reopen.
