Why the gap matters

Baby molars hold precise space for the premolars developing underneath them. When one is lost early — to decay, injury, or extraction — the teeth on either side begin drifting into the gap within months. The permanent tooth then erupts into insufficient space and comes in crowded, rotated, or impacted below the gum.

Regaining that space later means moving teeth back with braces, sometimes with extractions elsewhere to balance the arch. Holding it costs a fraction of that.

The types

Type	Where it is used
Band and loop	A single lost baby molar; a band on one tooth with a wire loop resting against the next
Lower lingual arch	Multiple losses in the lower arch; a wire running behind the lower teeth between molars
Nance or transpalatal arch	Upper arch, using the palate or a cross-arch wire to hold molar position
Distal shoe	A second baby molar lost before the first permanent molar has erupted; guides that molar into place

Living with one

They are cemented in and not removable by the child. The first few days feel strange and then are forgotten. Sticky sweets and chewing gum are the main risk, because they pull the appliance loose. Cleaning around the band matters — decay under a loosened band is the complication worth avoiding.

The appliance stays until the permanent tooth is on its way through, which is monitored at recall visits and confirmed on a radiograph. It is then removed in a short appointment. Children being monitored in this way are seen on free recall visits at this practice until the timing is right for whatever comes next.
